Medication Management

Did you know approximately 200,000 older adults are hospitalized annually due to adverse drug reactions? Older adults discharged from the hospital are usually on 5 or more medications. The number of prescribing providers adds to the complexity of managing one’s medications. This increases the risk of medication errors.

Reasons to consider medication management:
• You have recently been hospitalized for an illness that required medication
• You have experienced side effects from one or more of your medications
• You take four or more medications
• You have trouble sorting and/or remembering your medications
