Falls Intervention

The occurrence of falls increases as we age. Falls are also one of the major reasons for hospitalization. As part of our Quality Assurance Program (QA) South Shore Home Health Services, Inc. instituted the STAR program in order to help our patients avoid these painful and debilitating occurrences.

S – Safety
T – Transfer
A – Ambulation
R – Risk

We discovered that 83% of our patients were at risk for falls. Through patient and aide education, medication reviews, and agency/physician communication we have been able to minimize falls and their injuries that result significantly in 2009. 90% of our patients at risk for falls DID NOT FALL!
